These formulary brand name drugs are selected to meet patient needs at lower cost.

Medications Requiring Prior Authorization for Medical Necessity: This list is applicable to the NALC Health Benefit Plan Formulary Drug List with Advance Control Specialty Formulary®. This list of medicines will not be covered without a prior authorization for medical necessity.
