Important Information About Your Benefits

Below is information concerning your Plan’s deductible and out-of-pocket maximum:

CDHP Deductible and Out-of-Pocket Maximums

Annual In-Network Deductible: $2,000/person ~ $4,000/family
Annual Out-of-Network Deductible: $4,000/person ~ $8,000/family
Annual In-Network Out-of-Pocket Maximum: $6,600/person ~ $12,000 family
Annual Out-of-Network Out-of-Pocket Maximum: $12,000/person ~ $24,000 family

When you enroll in the CDHP during open Season, we give you a Personal Care Account (PCA) in the amount of $1,200 for Self Only, $2,400 for Self Plus One, or $2,400 for Self and Family. If you join at any other time during the year, your PCA for your first year will be prorated at a rate of $100 per month for Self Only or $200 per mont for Self Plus One or Self and Family for each full month of coverage remaining in that calendar year.

Important Notice: When you get emergency care, use an out-of-network air ambulance provider, or get treated by an out-of-network provider at an in-network hospital or ambulatory surgical center, you are protected from surprise billing or balance billing. “ Surprise billing” is an unexpected balance bill as defined by a new federal law called the No Surprises Act. This can happen when you cannot control who is involved in your care—like when you have an emergency or when you schedule a visit at an in-network facility but are unexpectedly treated by an out-of-network provider.
